Here are the details on this position. Your role in our mission
- Lead and oversee a team of care coordinators, including
providing guidance and support in the day-to-day work of care
coordinators. Provide input into performance evaluations
- Assist management in care coordination quality and monitoring
performance standards
- Supports care coordinator in escalations
- Collaborate with members/legal representative, health plan, and
providers, community-based organizations (CBO), interpreters and
others in accessing dental care
- Work with members and providers to coordinate necessary dental
and medical care
- Leverage a variety of resources, or combination of resources,
in order to assist the member in coordinating care
- Remove barriers to care such as assisting with appointments,
transportation, language assistance
- Identify members with special needs or other identified target
populations that are eligible for case management
- Monitor members are actively receiving appropriate
care/scheduled care and complete their goals
- Maintain and record decisions, actions, and outcomes for
members receiving care coordination
- Set up tasks and reminders follow up on missed appointments
What we're looking for
- A minimum of three (3) to five (5) years of experience in a
relevant Care Coordination position in the healthcare or insurance
industry with Medicaid and/or Medicare knowledge -is strongly
desired
- Previous lead or management experience in monitoring
performance standards of a team and supporting client
escalations
- Clinical and/or Dental health experience is preferred. Strong
knowledge of dental treatment authorization request processes,
procedures, and documentation is preferred.
- Professional or personal experience supporting individuals with
disabilities, chronic illnesses, or experiencing homelessness
preferred
- Effective verbal and written communication skills, exceptional
interpersonal, customer service, negotiation, and problem-solving
skills
- Exceptional attention to detail and strong organization skills
with the demonstrated ability to manage multiple responsibilities
and prioritize tasks related to project timelines to meet
schedules, client deadlines, and delivery deadlines
- Familiarity with care coordination methodology and the ability
to work independently and in a team setting
- Ability to build relationships with different types of people,
including clients, caregivers, family members, community-based
organizations, and members of the health care team
- Strong understanding of cultural -and linguistic
competency
- Education and experience: Registered Dental Hygienist (RDH),
BA, LMSW/LCSW/LMHC or RN/LPN degree preferred What you should

- Must be a resident of California
- Ability to work in a hybrid environment (1-2 times a week) and

The pay range for this position is -$48,400.00 -- -$69,200.00 per
year, however, the base pay offered may vary depending on
geographic region, internal equity, job-related knowledge, skills,
