We're looking for someone to join our team as a Health Plan
Creates and manages a dietary management program for managed
care cases in conjunction with other care management team members.
Creates and manages a dietary management program for Health Plan
population as it relates to dietary prevention of chronic diseases
- Creates and manages a program for member education for select
groups such as diabetics, congestive heart and renal failure
patients, obese patients, and others as indicated upon population
review and identifications of high-risk/complex care management
cases. Provides member education in dietary primary prevention to
our member population. Acts as a consultant to the care managers as
- Develops and documents nutritional care plans in conjunction
with member and care team. Works with treating physician and member
towards effective outcomes.
- Evaluates outcomes of nutritional care for various member
types. Re-evaluates care plans, as needed, and makes appropriate
adjustments in conjunction with member and physician.
- Documents interventions and outcomes in medical management
software system. Assesses patient and family educational needs and
possible barriers to learning and implementation of a nutritional
care plan. Develops plan(s) to overcome barriers in creative manner
and then implements with team members.
- Provides occasional in-service sessions for the care managers
or other team members on relevant topics. Assists with analytics
review of claims data from varied sources relative to potential
patient needs and program development.
- Performs other job-related duties as assigned.
You'll be successful with the following qualifications:
- Bachelor's Degree or equivalent (some certifications will
require Bachelor's): Required
- Master's Degree: Preferred
- Clinical experience in an inpatient/outpatient setting that
includes patient education: Required
- Registered Dietitian: Required
