Health Care Manager for a travel nursing job in Clearlake,
California. Job Description & Requirements - Specialty: Care
Manager - Discipline: RN - Start Date: 03/16/2026 - Duration: 10
weeks - 40 hours per week - Shift: 10 hours, days - Employment
Type: Travel description: /n/n Provides excellent patient care by
assisting in collaboration, development, implementation, revision
and reporting of the case management program. Acts as a liaison
between the patient, family, nurse, physicians, multidisciplinary
team and patient's healthcare benefactor to optimize outcomes.
Serves as a consultant to the healthcare team on specific patient
items. /n/n requirement: /n/n Day Shift, 40 hours per week,
coverage area is Lake County, CA; mileage (government rate) is in
addition as travel is needed to see patients. THIS IS FOR HOME
HEALTH NOT HOSPITAL. Home Health experience is A MUST/REQUIRED.
MUST have HCHB software experience is required. Must have OASIS
experience.Education and Work Experience:Bachelor's Degree in
Nursing (BSN): PreferredExperience in a care management role:
PreferredLicenses/Certifications:Registered Nurse (RN) licensure in
the state of practice: RequiredCase management certification:
PreferredBasic Life Support (BLS OR HS-BLS OR RQI BLS)
