Overview
Optage, a division of Presbyterian Homes & Services -is seeking a licensed nurse (LPN or RN) to join -its team -as an Intake -Service Coordinator. This is a full time benefit eligible position reporting -to the Optage -Administration and Reporting Manager and clinical management designee. - They will work collaboratively with Optage staff, site and community staff, and supporting outside agencies as designated and according to the focus of their activities. Hours are Monday through Friday 8:30am-5:00pm.

Responsibilities
The Intake Service Coordinator is responsible for performing the new patient intake procedures and responsible for identifying appropriate candidates for services. -This is a part of the complete intake process for Optage service lines. This includes but is not limited to initial fax, email or phone call from the customer, best first impressions, discovery of the customers needs, gathering client orders, -setting up the next steps and securing the end result that the customer was looking for. The Intake Service Coordinator is responsible for the provision of complete and accurate client information for all service lines. This includes getting information to the clinicians in a timely manner, prior to admission, to ensure a successful admission for each client, that is consistent with regulations, and meets each client's specific needs. Providing excellent customer service and public relations is also needed to assist in the maintaining efficient operations within Optage. Responsibilities include data entry, database records management, and other duties as assigned.

Qualifications

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) currently licensed with the State Board of Nursing in state(s) in which he/she practices
  • Experience in homecare, hospice, senior housing, or health care related environment.
  • Must have flexibility, personal integrity, and ability to work effectively with clients, residents, coworkers, family members, visitors, and the general public.
  • Must be able to work independently and with minimal supervision.
  • Demonstrated computer proficiency including Microsoft Office and other common computer programs.
  • Must be able to use lead management software, maintain detailed accurate up to date confidential records. -
  • Must be a team player, willing to take directions and accept responsibility for meeting specified objectives.
  • Must be good at developing and building relationships, excellent first impressions, presentations of service lines, and securing the customers services needed.
  • Must understand the basic principles of marketing and sales.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers. -
