NURSING FACILITY (SNF) POSITION:
- Receive admission and / or transfers to the unit
- Initial and on-going systematic patient assessment
- Timely and accurate documentation
- Interpret assessments / diagnostic data including labs and
telemetry
- Promote continuous quality improvement
- Respond to life-saving situations, using nursing standards and
protocols for treatment
- Act as a patient advocate while providing education and support
to patients and their families
- Demonstrate excellence in customer service and public relations
to promote a positive professional imageCREDENTIALS REQUIRED:
- Min 1 year of recent Registered Nurse RN experience in the
LTC/SNF setting Required
- Registered Nurse degree from an accredited institution and
active FL RN license Required
- Current CPR/BLS certification, Physical and TB/PPD
Required
- Verifiable work history and professional references Required
DUTIES: Under the supervision of the Clinical Supervisor, the
Primary Care Clinician (PCC) is an experienced RN or therapist who
is responsible for the overall management of approximately 25
patients including the provision of direct care with a designated
visit productivity standard. This position plans, organizes, and
directs all patient care services for patients in the assigned
caseload in accordance with current standards and regulations,
ensuring the optimal degree of quality care is maintained.
Responsible for coordinating the interdisciplinary team to
implement the established plan of patient care and ensuring
effective management of visit utilization and control of expenses.
May assist in agency wide orientation, competency evaluations and
consultation to clinical staff related to specific expertise as
requested by Clinical Supervisor. MINIMUM POSITION REQUIREMENTS:
Knowledge: Thorough understanding of home care and/or hospice
services, Title XXII, Medicare Conditions of Participation, JCAHO
home health and/or hospice accreditation requirements, QI
outcomes/benchmarking, infection control; good understanding of
reimbursement patterns for Medicare and other payors; good working
understanding of budget and other financial reports. Clinical
competency that meets and exceeds testing levels on a regular
basis. Special Skills/Equipment: Self-starter with a high degree of
initiative, motivation, flexibility, energy and creativity. Ability
to form harmonious working relationships with internal and external
customers. Demonstrated leadership and negotiation skills and the
ability to manage resources for patient care in a cost effective
manner. Excellent collaborative and problem solving skills with
customers and management. Ability to exercise independent sound
judgment in planning and providing patient care. Access to a
reliable automobile. Primary Job Responsibilities: --- utilizes
effective physical and psychosocial assessment skills, performs and
documents multi-system assessments including OASIS --- develops all
initial and recertification plans of care and ensures timely
completion to meet regulatory and payor timeframes --- seeks
appropriate and timely consultation for insufficient progress
towards patient identified goals --- works collaboratively with
other clinical and clerical team members to meet patient plan of
care and/or agency mission and objectives --- timely notification
to Clinical Supervisor of emergencies and/or unexpected or
significant changes in the patient medical condition ---
demonstrated knowledge and appropriate utilization of internal and
community resources to adequately meet patient needs Quality
Management (includes documentation and regulatory standards) ---
completes all admission paperwork in a timely manner including
consents, billing and clinical forms per agency policy and
procedures, including OASIS; competently uses agency designated
documentation tools, including computerized equipment as directed.

completion of all required discharge paperwork --- demonstrates

(includes productivity, HHRG rating, supply management, utilization
review) --- completes physician orders for all billable patient
supplies and equipment --- completes required billing requisitions
